Job Description

DHS is comprised of three divisions: Division of Aging Services, Division of Child Support Services, and the Division of Family and Children Services.

The Georgia Division of Family and Children Services (DFCS), Placement and Permanency Section have the unique opportunity of establishing a state level care coordination treatment team. Under general supervision, 14 Care Coordinators will work in two teams of 7 and report to their respective supervisor (Systems Management or Clinical Team) and administrator. The Care Coordinators are responsible for monitoring and partnering with local DFCS staff, private and public providers/resources to manage and oversee the quality of care provided to children in foster care including but not limited to Amerigroup managed care, promotion of family-based care, prevention of unnecessary placements through evidence based practices, service delivery and community mental health services.

Systems Management Team
The Systems Management Supervisor will supervise 7 statewide Care Coordinators. The Systems Management team will be responsible for providing a high level skill set and expertise on all interagency and statewide networks, assess systematic gaps and vulnerabilities within the child welfare continuum of care and look to minimize those deficiencies through effective partnering with local DFCS staff, private/public providers and resources.

Core Competencies:

Possession of a valid driver’s license and consistent access to an insured vehicle is required as a condition of employment.
Excellent project management skills.
Demonstrated ability to work effectively with limited supervision.
Ability to work effectively at multiple levels of the organization and with multiple project teams.
Excellent oral, written, presentation and interpersonal communication skills.
Strong proficiency in the use of Microsoft Office Suite and/or standard software applications typically used in a corporate office environment.

Job Summary & Responsibilities

Actively support, represent and extend the mission of the Division through direct services, customer contact, and all other assigned functions.
Resolves inquiries, complaints or concerns regarding complex cases.
Coordination with Office of Provider Management from our shift from congregate to family based care.
Coordination with State agencies to increase placement options and services.
Execution and reporting for Program Improvement Plan.
System(s) training, policy development and review.
Development of reports for data tracking and monitoring.
Manages care coordinator workload and unit coverage.
Track and monitor program effectiveness by coordinating with team on quality initiatives and expectations.
Evaluate and communicate to care coordinators opportunities for care intervention to close the gap in care for improved outcomes.
Creates clear defined expectations, procedures, goals and incentives for each individual within the team.
Ensures staff orientation, training, and ongoing support to new and existing care coordination staff.
Provide support to after-hours and weekend emergency care coordination schedule.
Other duties deemed necessary for effective and efficient unit operations.
